Need Paypal Checkout Form Created

We need a shopping cart checkout form created and embedded into our website on our page

There will be only about 10 items needed to be available for selection to purchase. This is to purchase registration not a physical item, so when need the steps to be as follows, please see the steps listed below and the attached graphics for reference:
Step 1> Items
This is where they can select the classes they want, they need the ability to add quantities and add to cart.
Step 2> Cart
this is where they can review there cart and remove items or proceed to checkout
Step 3> Checkout
This is where they will out all there contact information, please reference the screen shot for all required fields, captcha will need to be included at the bottom of this form to avoid spam. From here they will select paypal as there payment method or CASH and they will then complete the form and click continue.
Step 4> Confirm
will be where they review their listing and there information and there selected course, they can then click confirm to continue. Once they click to confirm the cart will take them to paypal where it will have all the items in the cart and have an invoice to charge them accordingly requested a credit card to complete purchase.
Step 5> Confirmation
This will be the page paypal brings them to once they have paid displaying them there confirmation number and the course they purchased.
A separate email should then be sent to them generated from our website once they have completed the checkout sending them a copy of their confirmation page information.
Please reference the screen shots for reference, need this completed within 3 business days once selected. If not completed within specified time frame a complete refund will be provided. The entire form must be embedded into our page listed above. You must create and test the form on your server.
